Teen Wellness Staff FAMILY NURSE PRACTITIONER MHC Healthcare will have a Family Nurse Practitioner on site to treat students and staff members. All our FNP’s are certified in the State of Arizona. MEDICAL ASSISTANT Along with the FNP there will always be a medical assistant from the MHC Healthcare on site. She will check the student in verifying student’s information, doing vitals and always assuring the student that they are receiving completely confidential, quality health care. Parents can reach the Nurse Practitioner or the Medical Assistant by calling 520-682-4111 if there are any questions or concerns regarding that days visit. Please call the school your child attends for all other questions or appointments.How to schedule and appointment PARENT’S 1.Student packets at the beginning of the school year will have the “Consent for Treatment”’. If you did not fill out this form at that time. You may call the health office at your child’s school and request one to be sent home. 2.Return this form to your child’s school health office. (via student or mail) 3.The parent or the student can set up a time to be seen. Parents call the health office. Students may request a pass for this or go during a break to schedule a time to be seen. (Talk with your child’s school to find out their protocol)FEE’S There is a $15.00 charge for your students visit. You may send cash or check. If your student has AHCCCS or an HMO/PPO coverage AND is assigned to the MHC Healthcare, your student will not have to pay this fee. Only their HMO/PPO co- pay will apply for that visit. Please send check or cash for this co-pay. PHYSICAL’S School and Sports physicals are provided for a $20.00 fee. Your child must bring a check or cash with them to that scheduled visit in order to receive the physical. Please call our office if you need to make arrangements for payment prior to the scheduled visit. 682-3777 and speak to our Programs Coordinator Anna Anderson. MEDICAL INSURANCE If your student has health insurance the MHC Healthcare will attempt to bill that insurance. If the insurance does not pay for any reason, the visit will be charged to the Teen Wellness Program and the cost absorbed by the MHC Healthcare. Please provide insurance information. NO MEDICAL INSURANCE If your student is uninsured an AHCCCS/KidsCare representative will assist parents with filling out an application and getting it submitted to the proper agency. Our office can provide you with a Sliding Fee Scale to cover xrays and prescriptions needed during the interim of AHCCCS/KidsCare eligibility determination. Xray's are NOT covered under the Teen Wellness program therefore you will need to call and speak with one of our Eligibility Specialists to schedule your screening for the Sliding Fee Scale. MHC Healthcare is working with Marana Unified School district to reduce the number of uninsured students in our school district We believe that health care and education go hand in hand. A healthy student is a more productive student.
